George Mason Club Sports Forms
What do the forms mean?
Form A: Participant Info & Packet Instructions — Basic waiver and cover sheet to the whole packet. Provides overview to Form G and Form H.
Form B: Club Sports Waiver: Lets you know that cycling can be dangerous and that you know that, and the any photos they take they can use.
Form C: Club Sports Emergency Contact: Yes, your Emergency Contact… just that simple!
Form D: Behavior Accountability: Don’t be bad, don’t do drugs, be a good sportsman, don’t post stupid stuff on Facebook, Twitter, Tumblr, Vine (etc), and hazing is wrong.
Form E: Concussion Facts & Acknowledgement: This is why we ALWAYS wear helmets. Fact sheet regarding the things to look for if your buddy goes down on his/her head… initial lots of times after reading.
Form F: Buckley Amendment Consent: Get good grades (that’s why your at GMU in the first place) and GMU has the right to audit your grades at any time if you want to be in the club. Club Membership already requires you to have a 2.0 GPA (Undergrad) and 3.0 GPA (Grad) so this is already happened at the RSO level.
Form G: Proof of Medical Insurance: This is probably the toughest form in the package. If you have your own medical insurance then put your info on it. HOWEVER, if you are still being covered under your parents insurance, you need their insurance info here (as I suspect most of you will be). If you have NO INSURANCE, don’t freak out! GMU has coverage through the Student Health Services (http://shs.gmu.edu/insurance/). If you don’t have insurance, or can’t/won’t get insurance, you can still be in the club, you just cannot race or participate in OFFICIAL club riding events until you do. However, having this info on file (to include the ICE — Form C above) is good to have if you go down or get hit by a car during a ride!
Form H: Physical Form: The instructions for this are on Form A. Form A lets you know that GMU does not provide you medical insurance (you have to provide your own — see Form G) and that you need take this form — Form H — to the doctor and get a physical. GMU clinic does them, the Burke TARGET clinic does walk ins for $50 with a student ID — takes 15 minutes, there is also a clinic near the Burke Walmart (similar deal). If you haven’t had one in a while, then you need to have one done.
